Answers about the program, including eligibility, options, customer service and more.

  • How much insurance can I purchase through this program?

You can apply for $50,000 through $250,000 of coverage, in $50,000 increments.
  • Can I also insure my family members?

Yes, you can also apply for coverage on your spouse (through age 64) and your children (14 days through age 23).
  • Are lower non-smoker rates available?

You qualify for nonsmoker rates if you have not smoked cigarettes, cigars or a pipe, or used chewing tobacco, nicotine chewing gum or snuff during the 12 months before submitting an application for life insurance. The non-smoker rates do not apply to residents in the state of New York.
  • When is the coverage effective?

When your application is approved by the insurer and we have received your first premium payment, your insurance will become effective as of the first of the month coinciding with or following approval. However, if on that date you are not Actively-at-Work, you will not be so covered until the first day you are Actively-at-Work. However, should the Effective Date be a non-work day, insurance will still become effective on that date if you are otherwise Actively-at-work on the last preceding scheduled work day.
  • Do I have to meet with an insurance agent?

Issuance of this policy is handled over the Internet and the mail. You can review the materials in the privacy of your home and purchase your policy directly through the mail without meeting with an agent. You can, of course, talk to a licensed representative if you'd like. Please view the Contacts section for the toll-free number.
  • What if I have second thoughts after I apply?

You will have 30 days from the date of receipt to review the insurance certificate. If you are not satisfied with the terms of the certificate, simply return it to the Insurance Administrator and any premiums paid will be refunded in full, minus any claims paid.
Life Form Series includes SRP-1153, or state equivalent.
